Procedural Safeguards

Individualized Education Program

Please note that we retain students’ Individualized Education Program (IEP) files for only three (3) years post graduation. At the end of the third year, the IEP files will be destroyed (burned, shredded, or other permanent destruction methods). The adult student may visit the school to retrieve their IEP files in their entirety prior to the end of the third year.

Utah Parent Center

The mission of the Utah Parent Center (UPC or Center) is to help parents help their children, youth and young adults with all disabilities to live included, productive lives as members of the community.

The goal is to provide students (ages 14-24) with a smooth transition, holistic team approach, and individualized activities and services that are necessary and reasonable to help prepare for, obtain, and maintain employment.

Workforce Services Rehabilitation

If you have received special education services from Entheos Academy and have graduated from high school on or before June 18, 2018, Entheos Academy will be shredding the IEP files on Friday, September 17th, 2021.
