Policies and Procedures

Entheos Academy, in providing for the education of students with disabilities enrolled in its school, has in effect policies, procedures, and programs that are consistent with the Utah State Board of Education Special Education Rules (USBE SER) as described in the Policies and Procedures Manual.

Procedural Safeguards

The Individuals with Disabilities Education Act (IDEA), the Federal law concerning the education of students with disabilities, requires schools to provide parents of a child with a disability or adult students with disabilities with a notice containing a full explanation of the procedural safeguards available under the IDEA and U.S. Department of Education regulations.

Individualized Education Program

Please note that we retain students’ Individualized Education Program (IEP) files for only three (3) years post graduation. At the end of the third year, the IEP files will be destroyed (burned, shredded, or other permanent destruction methods). The adult student may visit the school to retrieve their IEP files in their entirety prior to the end of the third year.

The mission of the Utah Parent Center (UPC or Center) is to help parents help their children, youth and young adults with all disabilities to live included, productive lives as members of the community.

Carson Smith Scholarship

If you are the parent of a child with an IEP enrolled in a public school, the district is required by Utah Code Section 53A-1a-704 (10) to inform you of the availability of a scholarship to attend a private school through the Carson Smith Scholarship Program.

USBE Special Education Services Scholarship

Vocational Rehabilitation

Vocational Rehabilitation provides Transition Services to eligible students with disabilities as they transition from high school to adulthood and employment. The goal is to provide students (ages 14-24) with a smooth transition, holistic team approach, and individualized activities and services that are necessary and reasonable to help prepare for, obtain, and maintain employment.

Workforce Services Rehabilitation

If you have received special education services from Entheos Academy and have graduated from high school on or before June 18, 2018, Entheos Academy will be shredding the IEP files on Friday, September 17th, 2021.
